About the Survey


Your participation in this study is entirely voluntary. By signing the consent form, you are agreeing to participate in this study. You do not waive any of your legal rights.

You may choose not to participate or you may withdraw from the study at any time. You will continue to be able to receive all services available to you through the breast screening program, the BC Cancer Agency, and your regular doctor. Information gathered from this study may be used by the SMPBC to design and implement a breast cancer risk estimation model in the future.


Your name will not be associated with information collected from this study. You will not be identified in any reports of this research. All of the collected survey information will be tracked by using only your study identification number. The investigators and the data collection / analysis staff will not know your identity.


No risks are expected from participation in this study. However, it is possible you may experience discomfort while recalling cancer history in your family.
